Senior CommUnity Care is a new health care choice for older adults on Colorado's Western Slope. Senior CommUnity Care is a provider of the Program of All-Inclusive Care for the Elderly (PACE) and is the first to operate in Western Colorado.
Senior CommUnity Care PACE is for:
Older adults who want to live at home but need support
Caregivers caring for older adults living at home
Health professionals who want to help older adults living at home with a complete care system
As a PACE Provider, Senior CommUnity Care lets people live independently with a high quality of life in the community. Senior CommUnity Care and PACE provide health care in the home and community to older adults who would otherwise live in long-term care nursing homes. Learn more about PACE in our Services section.
Senior CommUnity Care is a service of Volunteers of America.
